Failure to complete the request form correctly may mean your request is returned. The Request Form should be completed for all requests for CD information whether company specific or statistical data. If the request is for company/individual specific information please complete one form for each company or individual, this does not apply where there are several trading names for the company or individual (however, all known names should be listed to assist the database search). How to complete the Request Form for Consumer Direct Information? The OFT will also take into account the provisions of Part 9 of the Enterprise Act 2002 (‘EA02’) in deciding whether or not it is able to supply the information requested.
